In consideration of your accepting this entry, I acknowledge that my participation in the PWHS

Band on the Run 5k Run/Walk involves a risk of injury, including bodily injury, and assume

the risk for same. On my own behalf and on behalf of my heirs and legal representatives

and to the fullest extent permitted by law, I hereby release and discharge PWHS Music

Boosters, Inc., Port Washington-Saukville School District and all event sponsors and their

respective directors, officers, employees, affiliates, members, agents and representatives,

of and from any and all liability for injury, death, or damages and/or any other claims,

demands, losses or damages, incurred by me in connection with any aspect of the 5k run

and walk. Further, I hereby grant full permission to any and all of the foregoing to use my name

and likeness and image, as well as any photographs, videotapes, motion pictures, recordings and

any other record of this event in which I may appear for any legitimate purpose in advertising and

promotion for such broadcast and reuse. I understand that the race may be canceled or postponed

due to weather or other similar causes. Fees are non-refundable and non-transferable.
